We are looking for people to enroll in a diabetes program that is designed around your needs and your lifestyle.  Your participation will not only give you access to care from Zócalo Health physicians and promotores but your feedback and ideas will help us shape the next generation of Latino diabetes care.  For Latinos, by Latinos. We have a higher risk of developing Type 2 diabetes which leads to us  being sicker and having to spend more on health care costs.  Zócalo Health exists to change that and we are looking to you to help us. We understand that our people face language barriers, mistrust, cultural misalignment, and limited access to health care services. By working directly with our community to provide services in your culture and in your language we can better address the factors that are keeping you from living your healthiest life.

Do you know someone who has diabetes? Are you concerned that you could have pre-diabetes or diabetes?

There is truth in knowing your risk so you can do something about it. Understanding your risk and learning about how to manage it can help you take control of your health. We are here to help you!

Take 5 minutes to determine if you have low or high risk for pre-diabetes. If you are at high-risk for pre-diabetes you can enroll in our management program to give you access to resources and a care team to help you reduce your risk over time.
